IT Surplus Property Service Description and Forms
Any supplies and equipment owned by the State of Nebraska that are no longer needed in the normal course of business should be disposed of via the surplus property process, regardless of the condition. OCIO Logistics, in conjunction with the team who owns the submitted property will coordinate to ensure that equipment is disposed of in accordance with State of Nebraska policies/procedures. Failure to follow procedures outlined in the Surplus Property Service Description (pdf) may result in a delay in processing time and/or hourly charges.

Media Destruct Policy and Forms
Media possession must be documented for all phases of destruction. Each Data Center will have an OCIO Media Destruct Form that will stay with the hard drive, through destruction. Each Data Center will have an OCIO Media Destruct Log, which will be reconciled quarterly, by the OCIO Compliance Team.
